Please Read Carefully. I do not accept insurance in my practice. Our sessions may be covered by your policy’s provision for “out of network” mental health/behavioral health providers. At your request I will create an invoice at a mutually agreed upon frequency that you can submit to your insurance for out of network reimbursement.
I will assist you in any reasonable way so that you can submit to your insurance carrier for “out of network” coverage if you choose this option. Every policy has its own terms and coverage provisions. I am not able to contact or negotiate with insurance companies on your behalf.
Payment is due at the time of service unless otherwise specified.
I accept payment via cash including Zelle and Venmo, check, all major credit cards, or an HSA card. My time is billed as follows as of January, 2022:
Initial Phone Consultation (15 minutes): Free
Initial Session (75 minutes) $175
Individual Session (50 minutes): $150
Missed Appointments: Full Fee
Cancelled Appointment without 24-hours notice Full Fee
In some circumstances involving significant financial hardship, I may be able to offer a sliding fee, or a payment plan, depending on whether or not my limited number of sliding fee slots are already taken.
Twenty four (24) hour notification is requested for all cancellations of appointment time reserved. If you anticipate even a potential problem keeping a reserved appointment, please call to let me know. Should you cancel with less than 24 hours notice, I will do my best to fill the appointment hour. If I am unable to do so, you will be responsible for the full fee for the time reserved.
